On the evening of April 9, Jamal Phelps didn't show up to roll call. Finding this out of character, supervisors and fellow officers went to his apartment, finding Phelps on the floor and completely incapacitated.

Police were called to Holston Valley Medical Center after staff observed two young children "crawling around in the hallway....eating food out of the floor." Upon arrival an officer located their mother "passed out" in a chair.
